Description
Job Summary Baylor Scott & White Health is seeking a detail-oriented Clinical Health Maintenance Coordinator to support preventive care initiatives by identifying care gaps, coordinating patient outreach, and ensuring accurate clinical documentation. This role partners closely with providers and care teams to improve patient outcomes through proactive health maintenance, chronic disease management, and timely preventive services.The ideal candidate has experience working in a clinical setting, is proficient in Epic, understands medical records and health maintenance workflows, and enjoys collaborating with multidisciplinary teams to deliver exceptional patient care.
Schedule
• Monday - Friday
• 7:00 a.m. - 3:30 p.m.
• Some flexibility with work hours based on clinic needs.
Essential Functions
• Review Epic work queues, patient registries, and health maintenance reports to identify patients due for preventive screenings, chronic disease management, immunizations, and other clinical services.
• Utilize electronic medical records (Epic) to review patient charts, verify documentation, and coordinate appropriate follow-up care.
• Collaborate with physicians, advanced practice providers, nurses, and clinic staff to ensure patients receive timely preventive and chronic care services.
• Conduct patient outreach by phone and other approved communication methods to schedule appointments, address care gaps, and reinforce provider recommendations.
• Support transitions of care by monitoring patients discharged from hospitals or emergency departments and coordinating appropriate follow-up with primary care providers.
• Accurately document patient interactions, care coordination activities, and clinical information within Epic.
• Assist patients with referrals, scheduling, community resources, and navigating the healthcare system.
• Ensure accurate medical record documentation while maintaining patient confidentiality and HIPAA compliance.
• Promote exceptional customer service by addressing patient concerns with professionalism, empathy, and compassion.
Key Success Factors
• Experience working in a clinical healthcare environment.
• Working knowledge of preventive care guidelines and health maintenance workflows.
• Proficiency with Epic electronic health record (EHR) and medical documentation.
• Experience reviewing medical records and identifying care gaps.
• Strong organizational skills with excellent attention to detail.
• Outstanding verbal and written communication skills.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Excellent customer service and patient engagement skills.
• Strong critical thinking and problem-solving abilities.

Minimum Qualifications Education
• High school diploma or GED required.
Experience
• Minimum one year of experience in a clinical healthcare setting preferred.
Preferred Qualifications
• Experience working with Epic electronic health record (EHR).
• Experience reviewing medical records and clinical documentation.
• Experience supporting health maintenance, preventive care, or population health initiatives.
• Experience in primary care, ambulatory care, care coordination, or medical office operations.
• Knowledge of preventive screenings, chronic disease management, and quality measures.
• Certified Medical Assistant (CMA) or equivalent clinical certification preferred.
